Can the Wada test be replaced by fMRI and other techniques ? DEBATE

چکیده
The Wada test is typically used for identification of the languageand memory-dominant cerebral hemisphere. The test is considered the “gold standards”—a term implying that the evidence it furnishes is trust worthier than evidence from other methods. The advent of noninvasive functional neuroimaging now raises the possibility of replacing this “gold standard” pro-vided it offers equally trustworthy results. It will be argued here that these methods, namely functional magnetic resonance imaging fMRI, magnetoencephalography (MEG), and transcranial magnetic stimulation (TMS) do provide equally trustworthy results, and that therefore they may replace the Wada test in many if not most cases.

Using functional magnetic resonance imaging (fMRI) to explore brain function: cortical representations of language critical areas
Pre-operative determination of the dominant hemisphere for speech and speech associated sensory and motor regions has been of great interest for the neurological surgeons. This dilemma has been of at most importance, but difficult to achieve, requiring either invasive (Wada test) or non-invasive methods (Brain Mapping). In the present study we have employed functional Magnetic Resonance Imaging...

متن کاملBRAIN MAPPING IN NEUROSURGERY
Background and Aim: Brain mapping is a study of the anatomy and function of the CNS (central nervous system). Brain mapping has many techniques and these techniques are permanently changing and updating. From the beginning, brain mapping was invasive and for brain mapping, electrical stimulation of the exposed brain was needed. However, nowadays brain mapping does not require electrical stimula...
متن کاملWada-validated clinical fMRI reveals typical and atypical speech activation patterns in individual patients with epilepsy
Introduction The Wada-test, or intracarotid amobarbital procedure is used to determine the laterality of language dominance to avoid postoperative aphasia in patients undergoing surgery for seizures. The Wada-test is a combination of a cerebral angiography with the application of sodium amytal into one internal carotid artery at a time, thus effectively anesthetizing one hemisphere to test the ...
متن کاملIs the Wada test necessary prior to epilepsy surgery?
The Wada test was initially used to identify the hemisphere of language dominance prior to epilepsy surgery, but was subsequently applied to identify patients at risk of amnesia after temporal resection. The Wada test was later found useful in lateralizing the epileptogenic zone, predicting postoperative memory function, and predicting postoperative seizure control. The Wada test became more wi...
